At the Center for Family Support, we are committed to providing the highest level of Person– Centered care & planning. As a part of this process, we are conducting POM interviews available for the adults and children we service.
Personal Outcome Measures focus on the choices people have and make in their lives. The Personal Outcome Measures, developed by The Council on Quality and Leadership (CQL), are organized into 5 topic areas:
- My Human Security
- My Community
- My Relationships
- My Choices
- My Goals
Our interviewers are specifically trained on how to gather the best qualitative information; ensuring to capture a persons quality of life outcomes and plan meaningful supports. The Personal Outcome Measures focus on the choices people have in their lives and serves as a powerful tool for evaluating how our organization can assist to further individualize supports to help meet each measure. The POM interview allows CFS to learn more about the people we support and provide a better guide for our overall goal of person-centered excellence.
