The Center for Family Support, Inc. is committed to protecting the privacy and confidentiality of Protected Health Information, known as (PHI), about the individuals we support. As per current state laws and federal HIPAA Laws, unless CFS has received a specific written authorization from the individual being supported, their legal guardian or personal representative or applicable law otherwise requires or permits, a particular use or disclosure of protected health information may only be permitted for the purposes of (a) our agency’s treatment or habilitation activities, (b) our agency’s payment activities and reimbursement for services rendered and (c) other routine health care and business operations related to the welfare of the supported individual. Such information may be duly disclosed to other health care providers who render such services to the individual, unless otherwise objected to by the individual, their legal guardian or personal representative. Any requests for any information that would identify the supported individual should be brought to the attention of the program supervisor and the agency HIPAA Privacy Officer as per agency HIPAA Privacy policies.

The Center for Family Support, Inc. is grateful to our community of supporters and are committed to your privacy and our transparency.
To enable us to communicate with you, the Center for Family Support Foundation collects and maintains: your contact information (name, organization, address, email address, and phone number); and your donation information (amounts, dates, and payment methods).
Your contact and donation information are shared with programs to track contributions and facilitate communications, such as donation databases and email systems. These programs are mandated to treat this information confidentially and are prohibited from transferring the information elsewhere.
